Changing your contact details

To update details like your name, address, phone numbers or email address:

• in your NHS account(through the NHS website or NHS App)
• fill out a change of contact details form
• phone or visit the surgery from 8am to 6pm

Change of address

If you have moved to a new address, you can still remain registered with our practice, however we will not be able to offer the option of a home visit.  If this is something you would require, you will need to register with a new practice closer to your new address.

Changing your date of birth or name

You will need to show us proof that you have changed your name or date of birth. This could be a marriage or civil partnership certificate, deed poll or birth certificate.
